The Policy Band — Always Visible
Once you are logged in, a dark blue band appears at the top of every page showing your key policy information at a glance: the insured name, policy number, effective and expiration dates, insurance carrier, and your agent’s name, telephone number, and email address.
Six numbered tabs run below the policy band. As you complete each tab, its label turns green with a checkmark, and the progress bar beneath it fills in — so you can track your progress at a glance. You may select any tab to move directly to that section at any time.

Tab 1 — Business Information
Verifying your business details, mailing address, and contact information

Section 1 — Named Insured & Business Details
Named Insured & Business Details section of Tab 1
1Insured Name — Pre-populated from your policy.

3Entity Type (required) — Your business structure, such as Limited Liability Company, Corporation, or Sole Proprietorship.
4Description of Business Operations — Briefly describe what your business does. For example: “Automated car wash offering full-service interior and exterior cleaning.”
Section 2 — Mailing Address & Contact
Mailing Address & Contact section of Tab 1
1Mailing Address, City, State, and ZIP Code — Pre-populated. Confirm this is the address where you receive policy correspondence and correct any errors.
2Primary Contact Name and Phone — Verify the telephone number and add the name of the best person to contact about this validation.
3Email — Enter your email address so ISG can reach you promptly with any questions about your submission.
4Secondary Contact Name, Phone, and Email — Optional. Provide an additional contact, such as your bookkeeper or accountant, if you would like ISG to be able to reach them as well.
When FinishedSelect Save & Next → — available at both the top and the bottom of the page. The tab label will turn green with a checkmark.

Tab 2 — Business Location Information
Verifying your locations and entering reported sales
OverviewYour locations are pre-populated from information provided by your insurance company. Your main tasks are to verify each location and enter the reported sales for each one.
The Locations Table
Pre-populated location rows — verify each address and enter Reported Sales
1Review each row. Confirm the Physical Address, City, State, Class Code, and Class Description are accurate, and make any corrections needed.
2Reported Sales (required) — Enter your total annual sales (gross receipts) for the policy period at each location.
3Adding a location: Select + Add Location below the table to add a row for any location that is not listed. If you have comments regarding an added location, you may provide them in the applicable Policy Question comment box (Tab 3) or in the Description of Business Operations field (Tab 1).
4Sorting: Each column header includes an arrow that lets you sort the table — helpful if your business has many locations.
RequiredReported Sales must be entered for every location. Your submission cannot be completed without a sales figure for each location listed. If a location does not generate sales, entering $0 will suffice.
Navigating Between Tabs
The ← Back and Save & Next → buttons appear at both the top and the bottom of every tab, so you never have to scroll to move between sections.

1Select Yes, No, or N/A (Not Applicable) for each question. Every question must be answered — unanswered questions display an orange marker and the message “This field is required.”
2If a question calls for detail — for example, if you answer Yes — type your explanation in the Updated Information box beside it. Keep it brief and factual.
3As each answer is recorded, a green Saved confirmation appears beneath the response box.
Answered questions — the selected button is highlighted and each response shows a green “Saved” confirmation

Tab 4 — Provide Tax Return
Uploading your most recently completed business tax return
RequiredA completed tax return is required as verification of total gross sales for all locations. Please upload a copy of your most recently completed business tax return, including all schedules, in a single PDF file.
Uploading Your Tax Return
Indicate whether the file is password protected, select the document type, then attach your file
1Is this document password protected? (required) — Select No or Yes. If your file is password protected, we recommend saving an unprotected copy if possible so your submission can be processed without delay.
2Document Type — Select the option that best describes the return your business files — for example, Internal Revenue Service Form 1120 for a corporation. If you are unsure which option to choose, open the drop-down list and use the search feature to find the most appropriate document type.
3An upload row appears for the document type you selected. Select Choose File and attach your tax return. Use Delete if you need to remove the file and attach a different one.
4When your file is attached, select Save & Next → to continue.
TipIf you cannot find a matching document type, move to the Provide Supplemental Documents tab to upload an alternate document.
ImportantNew business with no filed return yet? Do not leave this tab blank — contact your ISG representative for guidance on acceptable alternate documentation before submitting.

Frequently Asked Questions & Pre-Submission Checklist
Common questions and a final checklist before you submit
Frequently Asked Questions
What if my business information is incorrect when I log in?▼
You can edit most fields directly in the portal. If key policy information — such as the policy number or insurance carrier shown in the blue band — appears incorrect, contact your agent or ISG representative before submitting.
What figure should I enter for Reported Sales?▼
Enter your actual annual sales (gross receipts) for the policy period at each location. Whenever possible, use the figures reported on your business tax return. If a location does not generate sales, enter $0. Contact your agent or ISG representative if you are unsure.
What format should my tax return be in?▼
Upload your most recently completed business tax return, including all schedules, as a single PDF file. If your accountant provided a multi-page PDF, upload the entire file. Contact your ISG representative if you have difficulty with your file.
My tax return file is password protected. What should I do?▼
Answer Yes to the “Is this document password protected?” question on the Provide Tax Return tab. If possible, we recommend saving and uploading an unprotected copy so your submission can be processed without delay.
Can I save my progress and come back later?▼
The portal saves your work as you go — you will see green “Saved” confirmations as your answers are recorded, and each completed tab turns green. We still recommend gathering your documents in advance so you can complete the validation in one sitting of about 10 to 15 minutes.
I answered Yes to a question — what should I write in the Updated Information box?▼
Keep it brief and factual. For example: “Added a delivery service in March 2025” or “Relocated to 456 First Avenue in January 2026.” ISG will follow up if more detail is needed.
What if I have no supplemental documents to upload?▼
If you provided a complete tax return and no additional documentation was requested, you may leave the Provide Supplemental Documents tab empty and select Save & Next to proceed to Review & Submit.
Whom do I contact if I need help?▼
Contact your insurance agent — their name, telephone number, and email address are shown in the blue policy band at the top of every portal page — or your ISG representative directly. Your Validation Notice also includes contact details.
